2. The reasons for the delay, if it is pending beyond the prescribed timeline.
3. The quantity of foodgrains and other items allotted against ration card no. ________ for the last six months.
4. The name and designation of the dealing official.
5. The expected date of issue or correction of the card.
I am a citizen of India. I am enclosing the application fee of Rs. 10 by Indian Postal Order (or as per the prescribed mode of the office). [If you hold a BPL card: I am below the poverty line and attach a copy of my BPL card; no fee is payable.] Kindly provide the above information within 30 days as required under the RTI Act, 2005.
