[Your Full Name]
[Full Postal Address]
[Phone / Email]

Replace every ________ and [bracketed] field with your own details before sending. The Rs. 10 fee is for Central public authorities; some States charge a different amount — check the Fee Calculator.

How to use it

  • Quote your UAN and Claim ID exactly as shown on the EPFO member portal.
  • A central RTI to EPFO can be filed online at rtionline.gov.in (EPFO is a central body).
  • Ask for the "deficiency memo" copies - it reveals exactly what is holding up your claim.
